  As of May 23, 2026, Reliant Behavioral Health & Community Services is proud to collaborate with Breakthrough Church North Campus, located at 481 Shelton Avenue, Hamden, CT 06514, on a community outreach initiative serving families, individuals with disabilities, and older adults in need. This partnership will help provide diapers, incontinence supplies, and other discreet essential care items ranging from pediatrics to geriatrics, including older children with disabilities, during  BreakThrough Church North Campus's monthly food pantry hours.   To ensure compliance with the Connecticut Diaper Bank guidelines, the   following documentation and procedures are required during the first diaper   pickup:

  • Valid, non-expired photo identification must be presented at the time of pickup.

  • Completion of the analytic demographic information form.

  • For newborns, infants, and toddlers, one of the following is required:

    • Birth certificate, or

    • Hospital discharge paperwork

  • Diapers may only be picked up by the child’s(ren) maternal, paternal parent, grandparent or verified legal guardian.

  • ​

  Failure to present required documentation may result in a delay or denial of service.
