To ensure compliance with the Connecticut Diaper Bank guidelines, the following documentation and procedures are required during the first diaper pickup:

Valid, non-expired photo identification must be presented at the time of pickup.

Completion of the analytic demographic information form.

For newborns, infants, and toddlers, one of the following is required:

Birth certificate, or
-
Hospital discharge paperwork

Diapers may only be picked up by the child’s(ren) maternal, paternal parent, grandparent or verified legal guardian .
Failure to present required documentation may result in a delay or denial of service.
